Observation

WAC 110-300-0460(2)- During the onsite monitor visit this licensor observed all the enrolled childrens files missing documents including registration forms, emergency information,consent of medical, permision forms, or any plans for special needs or individual needs of a child. This licensor also observed that one of the children in attendance did not have a file and another child's file who the provider said had disabilities only had the CIS form in the file and no other doucments or an ICP plan.
